Depending on the package chosen, from 259 MYR / 2 hours, to if I recall 380 MYR / 2 hours, *excluding* the extra tips.
For the extras, usually, 100 MYR for BJ, 200 ~ 300 MYR for FJ (depends on the girl - not all offer FJ).
HJ free & included in the massage itself (though not written explicitly on their sites).
